Enrollment and Technical Support

MyCharlie’s Virtual Assistant with AI technology and Client Services support outreaches to insured members using client’s white labeled messaging to introduce the AHC Health & Wellness Program.

Technical support is available 24/7 via the MyCharlie Mobile Application.

Risk Stratification and Engagement

AllHealth CHOICE proprietary and robust data analytics tool provides a comprehensive risk analysis. Risk Stratification is determined by a member’s health score or status of high, moderate, or low using a variety of methodologies including demographics, lifestyle behaviors, claims, and predictive modeling analyses. Stratifying members is an ongoing and dynamic process to ensure seamless delivery of customized action plans and focus on the member experience.

All members at all risk levels have access to the MyCharlie Mobile Application and the 24/7 Virtual Assistant powered by AI technology. Designed to engage, educate, and evaluate individual needs and allows members to have on-demand access to our interdisciplinary Population Health Team with frequency of touchpoints determined by clinical complexity and needs.

Additionally, all members will complete a personalized Health Risk Assessment (HRA) which returns a health summary to the member providing an intuitive visual of where they currently reside on the health continuum.

The MyCharlie Mobile Application and the 24/7 Virtual Assistant navigates all members on their personal healthcare journey by allowing connection to wellness applications and wearables for enhanced lifestyle behavior monitoring.

The Population Health Team, with member collaboration, develops dynamic and person-centered care action and education plans using Milliman Care Guidelines (MCG). 

Members with minimal health risks are considered low risk and will receive all services as outlined above to enhance their experiences and maintain a healthy lifestyle. Ongoing monitoring and engagement will allow our Population health team to be aware of any changes in risks to ensure each member is provided the right experience at the right time.

MODERATE RISK

Those members at the moderate risk level generally have one chronic condition and receive additional services including,

  • Bluetooth enabled Medical Peripheral Devices specific to the member’s chronic medical conditions to monitor and manage vital alerts.
  • Disease-specific, lifestyle behavior, health risks, and care pathways to develop person-centered care action plans and educational resources using Milliman Care Guidelines (MCG).
  • Medication reminders and adherence monitoring.
  • Care Coordination across the care continuum.
  • Tablet equipped by FirstNet®, built with AT&T, with reliable and secure connectivity if the employee does not have their own smartphone.

    HIGH RISK

    Members with the most complex clinical needs and multiple chronic conditions are considered high-risk and receive additional services including,

    • Bluetooth enabled Medical Peripheral Devices specific to the member’s chronic medical conditions to monitor and manage vital alerts.
    • Medical Therapy Management (MTM).
    • Medication reminders and adherence monitoring.
    • Disease-specific, lifestyle behavior, health risks, and care pathways to develop person-centered care action plans and educational resources using Milliman Care Guidelines (MCG).
    • Annual nutritional assessment with registered dietician. 
    • Tablet equipped by FirstNet®, built with AT&T, with reliable and secure connectivity if the employee does not have their own smartphone.

    Digital to Human Experience: MyCharlie Care


    In our experience, digital solutions are abundant, but often missing the support of human connection. The AllHealth CHOICE solution, MyCharlie Care, meets our members on the personal level with 24/7 Health Coach coordination. Through our HITRUST Certified mobile application/web browser, our Population Health Management Team receive real-time data via FDA approved, Bluetooth enabled devices. Real-time vital sign readings facilitate responsive symptom management, and the member engagement that reduces those critical and catastrophic events that cause a decline in healthcare claims. 

    The AllHealth CHOICE Care Management Team utilizes the predictive analytics reporting as well as the population health management report to stratify the population into groups by chronic condition and claims spend. The Population Health Management Report provides a comprehensive review of medical and pharmacy claims data allowing the care manager to combine claims data with clinical health risk data to group members into low, moderate, and high levels of risk for clinical care management services. 

    The Population Health Management Team consists of Nurses, Chronic Care Certified Professionals, Health Coaches, Pharmacists, Social Workers’, Certified Diabetes Specialists and Dieticians who engage the members in motivational interviewing and coaching techniques to address therapeutic lifestyle risk, well-being, and mental health issues. The team engages the member in person-centered goals and shared decision making to create a customized care action plan. Interactive discussions around personal health data, biometrics, vital statistics, and barriers to change as well as utilizing claims-based reporting on evidence rules and HEDIS measures work together to evoke change, promote adherence, and reduce catastrophic occurrences.
